    I found it out after starting another play through, Unconfirmed was the mission that didn't log correctly.

    @@Melody yes did every mission in order so I'll just try to run through it again. Unease p34nut would skipping a cutscene do it? I plan on doing it again but just not right now.

    I beat halo 5 guardians on legendary (by my self) a couple days ago. Even though I did this I did not receive either of the two achievements I should have. Anyone can check my service record and see this. Is there anything I could have done to mess this up?
