Kats shields were taken down by the radioactive explosion n in new Alexandria and she didn't put them back up because they would take time this proved to be her downfall as she was shot in the head.

 

Carter flew into a scarab and blew up though his death saved humanity

 

Emile was stabbed in the chest by a Elite and died ( you can see his body when you go up to fire the cannon )

 

Noble six held out for hours but in the end was overwhelmed by the sheer number of Elites

Non of the Human controlled planets visited by the Covenant were destroyed, however they were completely Glassed.  Exposure to great heat melting rock, sort of like Lava from a Volcano, or in other words Obsidium.  Black Glass.  Holy Charity has a room where the walls are lined with Glass from burned worlds.

Reach was the only world, I believe, that was not completely Glassed.  The Covenant are on a Holy Quest to gather as much Forerunner Technology as possible, and Reach is one of the places with Forerunner Tech.  Almost like Humans Holy Quest for the Grail, sort of...
