In the halo 4 level "reclaimer" there is a game-breaking glitch after the destruction of the large covenant carrier (2-3 times bigger than phantom. What happens is I get back onto the Mammoth to proceed and the environment is gone. The Mammoth drives on air for awhile and then the Mammoth also disappears. It is then impossible to move. I have not yet found a solution to this glitch.
