Ok so you have Reach inserted on startup of the xbox meaning that you are not actually going into games to.execute the disk. The only thing I can think of is that you may need to rewrite the Reach disk to your hard drive. just turn outo launch off and copy it over again. That should fix the intermitent bug. If not, reply back and.we can try something elae.
