The Director
1. Why does this happen? 2. How do you fix it? 3. Is it your internet connection? 1. It happens because someone is playing on a damaged disc, so their X-Box cannot read the map correctly. 2. You fix it by either taking care of your disc, or by downloading the disc to your hard-drive. (it's best to borrow someone's undamaged version of the disc to download) 3. To an extent. If your connection is too slow it will sometimes not be able to load the map on your console and you will get a failure to load. However, this rarely happens due to a connection. This happens when your disc is so damaged that your console cannot read all of the maps properly. However, if you download an undamaged disc to your hard-drive, it will load from your hard-drive rather than the disc. This means that unless your disc becomes completely unreadable (shame on you for letting that happen, btw) your x-box will load the undamaged version of the game from your hard-drive every time, preventing a failure to load. Mind you, it only takes one person in the lobby who is unable to load the content to cause a failure to load to happen, so spread the word.

fzdw11
if someone plays halo a lot they should download the game to console also DLC is saved onto a hard drive and still have load issues its the connection not CD Complications xP Hm. I don't ever remember having an issue with a load failure on DLC... but it could still be the CD, or in the case of DLC, a corrupt file. I was just playing with my sister-in-law and her fiancee last weekend, and couldn't play any of the Anniversary maps, yet I know they have them. Made them party leader and behold, the files got corrupted and they had to redownload the map pack.
The Director
if someone plays halo a lot they should download the game to console also DLC is saved onto a hard drive and still have load issues its the connection not CD Complications xP Actually, the DLC still has to be accessed through the disc, which if the disc is damaged will also make the DLC inaccessible. If you've got it installed, then it's all on your Hard Drive, and the disc is only used as verification that you own the game. And the only time that you'll get a load failure due to connection is if someone is about to lag out. lol The reason it happens because of a connection issue is because the player "times out", which usually means that unless their connection improves, they are going to lag out. Also, if it still happens frequently even though you have a good connection, it may be because of minor corruptions in the download that happened. Just delete and redownload to fix. The Director
Oh, also, if you get a failure to load on Halo 3 (bringing this up due to the playdate that happened today), then it can be one of two things. Either your disc is messed up, or you do not have the DLC. If your disc is messed up, clean it following The Directions (see what I did there? ;D) in this video. http://www.youtube.com/watch?v=D7gP3SnzbI4 If you do not have the DLC, or have it but it won't load, go to the X-Box Live Marketplace and either download or redownload the Map Packs. As stated in a previous post, occasionally during your download the DLC becomes corrupted, and will not play. If the DLC is already downloaded and redownloading does not work, try deleting it from your harddrive and then attempt to redownload it again. If THAT doesn't work, then give Microsoft Support a call.

Anyway, try hard resetting your Xbox. Don't put it in power down mode, turn it off entirely. If that fails, look at your internet connection, because that might be the problem too.
